ARBOR HEALTH ELDER SERVICE PLAN is a division of Harbor Health Services, Inc. (HHSI). A non-profit health care corporation, HHSI came into existence in 1985 with the merging of the Neponset Health Center and the Geiger-Gibson Community Health Center, the nation's first community health center. Currently, HHSI operates four health centers, a childrens' day care center, WIC programs, dental centers, and the Elder Service Plan, assuring the availability of primary health care services throughout the continuum of life.Continuing with its mission, HHSI developed the Elder Service Plan in Boston and part of the South Shore in 1996. The goal of the Elder Service Plan is to maintain elders in the community.

Elder Service Plan is funded through a joint contract between the Department of Health and Human Services, the Commonwealth of Massachusetts and the Elder Service Plan of Harbor Health.
