PPLICANTS MUST MEET THE FOLLOWING CRITERIA:

1. Be 55-years-old or older
2. Live in one of the following areas: Avon, Boston, Braintree, Brookline, Canton, Dedham, Milton, Norwood, Quincy, Randolph, Sharon, Stoughton or Weymouth
3. Have health problems that prevent them from living independently, qualifying them for "Nursing Facility Care," as determined by the State
4. Be able to live safely in the community, with additional supports

We accept Medicare, Medicaid and private payments.

(Enrollment is voluntary. You will be liable for costs not within the plan or outside of network for non-emergency care).
