HE ENROLLMENT PROCESS is as follows:- Call to obtain more information about the program. We can then discuss your particular situation.
- A home visit will be made by an Enrollment Specialist. During this time, the Medicaid application process will be started, if necessary.
- Site Visit 1: We will arrange for the potential participant to come to the ESP center for a formal screen by a nurse and social worker. The formal screen assesses whether or not the individual qualifies as needing "Nursing Facility Care." Family members are asked to participate in this visit. The Medicaid application process may be started during the first site visit.
- Site Visit 2: A second site visit will be scheduled for the potential participant to meet the health care team (doctor, nurses, rehabilitation staff, activities staff, etc.) This second visit will include time to observe the day center activities. The family is not required to attend.
- The following must be approved before the enrollment process can continue:
- The formal screen must be approved by the State.
- The ESP team determines that the individual will be safe in the community.
- Meet with an enrollment representative to do the "signing in" for the program. Enrollment into the program begins on the first day of the following month.
