- What is the difference between PACE and Elder Service Plan?
Elder Service Plan is part of PACE (Program All-inclusive Care for the Elderly), a nationwide model of care designed to maintain elders in the community, so they can live independently as long as possible.
- How does the continuum of care work at ESP?
The same team of clinicians manages each participant's care, whether in the home, a contracted assisted living facility, a contracted hospital, or a contracted nursing home. The enrollment specialist has a listing of contracted facilities.
- Can I keep my doctor and my specialists?
No, a PACE doctor will be provided who will coordinate your care. We have a listing of specialists that we use. Outside specialists are used on a case-by-case basis. Talk to enrollment staff if this is a concern.
- How many hours of home health aide services will I get if I stay at home?
This is determined by the team, family and participant. Due to the preventive nature of the program, participants usually receive daily service.
- How do you work with assisted living facilities?
We have a listing of contracted facilities. We cover the cost of the home-care component in these settings. You are responsible for rent. For more information, contact the Elder Service Plan representative.
- What do they do at the day-care center? Do I have to attend every day?
The Adult Day Health Center has recreational activities (ask us for our calendar). Our clinicians and rehabilitation staff are located on-site (one-stop shopping).
Each case is reviewed on an individual basis to determine the ideal number of days of participation per week, which is then discussed with the family and the participant. Some participants come for appointments only.
- What happens if my parent needs nursing home care?
ESP has contracts with specific nursing homes in the area. (Call us for a list of contracted nursing homes).
- What is the cost?
We accept Medicare, Medicaid, and private pay. For those eligible for Medicaid, there may be a monthly "spendown" due to financial criteria. The private payment or spendown amount is not affected by any change in the participant's health status. Please talk to an enrollment representative about your particular situation.
All ESP services are covered 100%.
- Where are the PACE programs in Massachusetts?
There are six PACE programs in the State, each of which covers its own geographic area. Call one of the sites listed below to see if your city or town is covered:
